A free tool for scoring and tracking workplace risks on a colour-banded 5×5 likelihood × severity matrix. Keep a full risk register, rate each hazard before and after controls to see the residual risk, and export the register as CSV or the matrix as a PNG. Everything is saved on your device and runs offline.
How to use Risk Assessment Matrix
- Add a risk — Click '+ Add risk' to create a row in the register, then describe the hazard.
- Score likelihood and severity — Choose a likelihood (Rare to Almost certain) and a severity (Negligible to Catastrophic). The tool multiplies them into a score and colours it Low, Medium, High or Extreme.
- Add controls and residual risk — Write the controls or mitigation, then set the residual likelihood and severity to show how the risk drops after those measures.
- Review the matrix — The 5×5 matrix counts how many risks land in each cell, with a colour legend so you can see your highest priorities at a glance.
- Export — Use the header buttons to download the register as a CSV file or the matrix as a PNG image.
Frequently asked questions
How is the risk score worked out?
Score equals likelihood multiplied by severity on the 5×5 scale. Bands are Low (up to 4), Medium (up to 9), High (up to 14) and Extreme (up to 25).
What is residual risk?
It's the risk that remains after your controls are in place. You rate the residual likelihood and severity separately so you can show the reduction from mitigation.
Is my data uploaded anywhere?
No. Everything is worked out and saved entirely in your browser — nothing is uploaded — and your register stays on your device between visits.
What can I export?
You can export the risk register as a CSV file for spreadsheets, and the matrix as a PNG image to drop into reports or slides.
Tips
- Fill in controls and residual scores so reviewers can see the risk drop after mitigation, not just the inherent score.
- Export the PNG for safety reports and the CSV when you need the figures in a spreadsheet.
- Use the matrix cell counts to focus first on anything sitting in the High or Extreme bands.
